The office of Daniel J Leeman, MD was recently impacted by a data security incident that affected the personal information of certain of its current and former patients. We have no reason to believe that any personal information has been misused.

What happened?

We discovered unauthorized access to our computer network between March 15, 2024, and March 28, 2024. Upon learning of this unauthorized access, we immediately began an investigation to determine the impacted data. On August 5, 2024, the investigation determined that certain personal information of our patients was impacted by this incident.

What information was involved?

The personal information involved included the following: name, date of birth, age bracket, Social Security number, Tax Identification number, telephone number, email address, login, driver's license number, digital / electronic signature, financial account information, payment card information, Medicaid / Medicare number, health insurance policy-related number, medical provider name, patient account number, medical record number, medical history, medical treatment/procedure information, medical dates of service(s), prescription information, and diagnosis / clinical information. Please note that not all data elements were involved for each individual. To date, we are not aware of any actual fraud or identity theft instances involving individuals’ information. What we are doing.

We value and respect the privacy of personal information. We take the protection of your personal information seriously and are taking steps to prevent a similar occurrence. As a result of this incident, we have worked with cybersecurity professionals to investigate, remediate this incident and prevent similar incidents in the future.
